Ordering Information

If you would like to order a sample license plate, please mail or fax a letter with your plate choices, name, address, daytime phone and check or money order payable to DMV or a Payment Authorization Form (ADM 205).

Souvenir Plates

Souvenir Plate

Anyone can order a souvenir plate in the Home Means Nevada design. Souvenir plates feature custom letter and number combinations printed in red and may not be used on a vehicle.

Charitable groups which sponsor a specialty plate may order souvenir versions of their own plate for fundraising purposes. They may also grant permission to other individuals. A letter from the sponsoring agency must be included with the request.

Any combination of up to seven letters and numbers may be ordered provided it is not offensive, does not conflict with Nevada standard plate numbers and does not violate any trademark or copyright.


Ordering Information

To order souvenir plates, please complete and mail or fax the following application. For mailed orders, include a check or money order payable to DMV or a Payment Authorization form. For faxed orders, fax the application and a Payment Authorization to (775) 684-4797.
